Location: This position will begin out of our Austin, Grand Prairie (DFW) or Houston office, and then will transition to a remote from home position after evidence of training.

Under the direction/supervision of an RN, contributes to the case management process by performing activities within the scope of licensure including, for example, assisting members with scheduling appointments, locating physicians,

screenings for potential needs using only yes or no questions with predefined responses, and other activities requested by an RN, who has overall responsibility for the case management plan for the member. No hands-on nursing required.

Primary duties may include, but are not limited to:

  • Assists in determining the physical and mental health status, needs and preferences of culturally, ethnically and socially diverse members based upon the collection of health related data.

  • Provides all information collected to the responsible RN, who verifies and interprets the information, conducts additional assessments, as necessary, and develops, monitors, and revises the member’s case management plan to meet the member’s needs.

  • Work is assigned by specific functions based upon the Case Management team’s needs and can be tracked by Primary Care Coordination cases or by secondary on RN’s cases.

  • The LVN/LPN decision making skills will be based upon the current needs of the member and requires an understanding of disease processes, terminology and the application of clinical guidelines as applicable and appropriate within current LPN/ LVN licensure and company requirements as overseen by an RN.

Qualifications

Qualifications:

  • Current unrestricted LVN license in applicable state(s) is required.

  • Minimum of 3 years of related health care experience; or any combination of education and experience, which would provide an equivalent background, is required.

  • Pediatric experience strongly preferred.

  • Ability to travel locally (minimal) within an assigned area is required.

  • Home Health experience preferred.

  • Bilingual Spanish preferred.

  • Proficiency in Microsoft applications and the ability to use a laptop computer is required.
